Res. No.__________Accepting the proposal and service agreement from McGriff, Seibels and Williams, Inc. for Specific Stop Loss Reinsurance for the City of Dothan’s Health Insurance Plan, as quoted by HCC Life Insurance Company.

Res. No.__________Renewing the contract with Blue Cross and Blue Shield of Alabama to provide and administer a Health Care Plan for City employees for a one-year period beginning October 1, 2022 through September 30, 2023 and accepting proposed administrative fees.

Res. No.__________Entering into an agreement with Justifacts Credential Verification, Inc. for a fee-based service for background checks required for employment with the City of Dothan for a three (3) year period.

Res. No.__________Entering into a renewal agreement with UL Verification Services Inc. for the use of Employee Health Clinic software and to provide maintenance and support at an annual cost of $15,120.00 for a three (3) year period, and appropriating funds for said agreement.

Res. No.__________Entering into an agreement with Telepak Networks, Inc. D/B/A C Spire to allow attachment of communications cable to City-owned utility poles.

Res. No.__________Entering into an agreement with Knology of Alabama, Inc. D/B/A Wow to allow attachment of communications cable to City-owned utility poles.

Res. No.__________Entering into a permit agreement with the Alabama Department of Transportation for the installation of a reducer, pvc water main, fire hydrant, flush valve, and tap and bore at 4929 South State Highway 134 for domestic water service.

Res. No.__________Approving a change order with L and K Contracting Co. Inc. for the 2021 Third Avenue Redwater Project to deduct $48,662.00 from the original contract amount, resulting in a final contract price of $826,077.00.

Res. No.__________Entering into an agreement with Axon Enterprises, Inc. for taser hardware, software, services, and warranties at a total cost of $174,600.00 for a four (4) year term.

30.

Res. No.__________Submitting an application through the Safe Streets and Roads for All grant program for the preparation of a city-wide equitable Safety Action Plan at a Federal/City cost share of 80/20 with City funds estimated to be $58,834.00.

31.

Res. No.__________Declaring various Christmas decorations as surplus and no longer needed for municipal purposes and authorizing the Leisure Services Department to donate said decorations to local municipalities.

Res. No.__________Declaring certain personal property as obsolete and no longer needed for public or municipal purposes and authorizing the disposal of said property by whatever means is determined to be in the best interest of the City.

Res. No.__________Awarding bids and approving other purchases over $15,000.00 by the City, and appropriating funds for said bids and purchases.
